(Toledo News Now) - A report from the Centers of Medicare and Medicaid services say the hospital is now in danger of being terminated from the Medicare program because of the incident.
The report says a nurse who accidentally disposed of a kidney during the transplant did not realize the organ was in the materials she removed from the operating room and then took to a "dirty utility room."
The report says poor communication, oversight and insufficient policies all played a part in the incident.
