
A troubled local nursing home is closing its doors after several run-ins with The Ohio Department of Health.
This will affect workers and residents working and living at Liberty Nursing Center of Toledo located at 2005 Ashland Avenue.
Back in August of 2012, the Ohio Department of Health notified the Liberty Nursing Center of its intent to revoke its license and close the nursing home.

At the time, Ohio's Long Term Care Ombudsman Bev Laubert said her program had a number of resources to help displaced residents.
At last check, there were 105 residents living in the facility.
